Here is my take on expanding the Damage By Level Tables in the Dungeon Masters Guide.

The Bonus is calculated by taking the average from the dice in the Low Column.

e.g. Between 37th-39th the 2d12 dice average 13. So 13 is the bonus.

If the dice remain the same for the next row on the Table, then the bonus is the difference between the previous row and the next row (in such cases round fractions up)

e.g. 37-39 has a bonus of +13, while 43-45 has a bonus of +16. Therefore the bonus for the row between those two is 15 (14.5 rounded up).

In cases where the dice suggest using more than 4d12s I have converted them over to d10s, simply because I think people will have more d10s than d12s.

To calculate the damage of Minions always use the minimum amount from the Low Column (Normal Damage Expressions).

e.g. A Level 50 Minion would deal 22 damage.

I would suggest that as default you use the Low Damage Column (Limited Expression) for Recharge 4 5 6, the Medium Column for Recharge 5 6 and the High Damage Column for Recharge 6.
